Liability Waiver

1. Assumption of Risk

I understand that exercise carries inherent risks including muscle soreness, sprains, fractures, cardiovascular complications, and even death.

I confirm that I am voluntarily participating with full knowledge of these risks and assume full responsibility for any injuries.

2. Medical Clearance

I affirm that I have consulted with a physician or have voluntarily chosen to participate without medical approval, understanding the risks.

I agree to notify my coach immediately if I experience unusual pain or adverse symptoms.

3. Release of Liability

I hereby release Ascencio Future Inc and its coaches from all claims, damages, or liabilities arising from my participation, including physical injury, property loss, or loss of life.

4. Full Disclosure Requirement

I agree to fully disclose all medical conditions, injuries, surgeries, and limitations. I understand that failure to disclose significantly increases my risk of injury.

5. Indemnification

I agree to indemnify and hold harmless Ascencio Future Inc from any claims brought by third parties arising from my participation in fitness activities.
